Custom Tattoo Booking Form

Hello! Thanks for your interest in getting a custom tattoo from me. Please read through this form carefully and provide as much info as possible before submitting, and I'll get back to you shortly. I only take on a handful of custom projects each month, and typically only take on projects that work within my existing style. Please list a few dates for both months listed above, in the event one month is fully booked. I tattoo at Geocity Tattoo in Bushwick, NYC, off the Morgan Ave L stop.

 

PLEASE READ BEFORE BOOKING

-Per New York state law, all tattoo clients must be at least 18 years of age. When you arrive for your appointment you will sign a consent form as well as a form with instructions for aftercare. Feel free to ask any questions during that time :)

-I tattoo Saturdays through Tuesdays, 11am-4pm

-My custom rate starts at $250. I have the ability to take limited custom requests each month on a case-by-case basis. I'll consider all custom requests received, however I will only take on designs that work within my existing style. If I don't believe I'm the best fit for your piece, I'll happily send you some artists in the area whose style is better suited for your idea :)

-Please note I only do blackwork. A non-exhaustive list of ideas/motifs that I work within: custom lettering/text, gestural linework, animals, florals, classical artwork. I do not typically take on hyper-specific portraiture, and ideally look for custom submissions that will allow me to have a bit of creative freedom.

-I will send you a draft of your custom piece in the week before your appointment. Feel free to respond with any additional corrections, however I ask that you keep this to one round of changes to be respectful of the time I spend outside of your appointment drawing. Once you send over your changes, I will finalize the design— we will have time on the day of your appointment to make final, minor tweaks, but ideally the design will be close to what you’re looking for by the day of your appointment. In the event of a more intricate/complicated piece, I may ask you to come in for a free consultation in person. so we can discuss the piece in more detail.

-I typically require a 50% deposit in order to book your appointment, which goes towards the final cost of your tattoo. For multi-session pieces, freehand pieces, or otherwise more involved work, this deposit amount may change or be split to cover multiple sessions. Your appointment is not booked until your deposit is recieved and confirmed by me. This deposit is nonrefundable, please make sure you are willing to follow through on your appointment before placing a deposit. Deposits will not be refunded once received.

-If you need to reschedule your appointment, please reach out more than 48 hours in advance. Cancelling/rescheduling within 48 hours of your appointment, or rescheduling more than once will forfeit your deposit, and you will be required to send another deposit to book.

-No call no shows will not be allowed to book in the future.

-I will reach out to you via email to accept your appointment and provide info on sending your deposit. I accept Zelle, Cashapp, Venmo or Paypal. The remaining balance on the day of your tattoo can be paid via Zelle, Cashapp, Venmo, Paypal, or in cash. Please avoid putting things like "tattoo" or "deposit" in the caption ("Thank you!" or emojis are great)

-Tips are not required but greatly appreciated, and range from 15%-25% if you had a great experience :)
