Hello! Thanks for your interest in getting a tattoo from me. Please read through this form carefully and provide as much info as possible before submitting, and I'll get back to you shortly. I'll be tattooing at Twice Rabbit Tattoo in Silverlake, LA.
PLEASE READ BEFORE BOOKING
-Per Califormia state law, all tattoo clients must be at least 18 years of age. When you arrive for your appointment you will sign a consent form as well as a form with instructions for aftercare. Feel free to ask any questions you may have during that time :)
-I am only booking flash pieces for May 12th, 13th, & 14th, 11am-7pm.
-My flash designs are all priced on their respective flash sheet, and start at $150 up to $300+ depending on size and detail. Most flash designs are repeatable, but I do retire designs once I've done them a handful of times--please be sure to check my available flash highlights on Instagram to see what is currently available. I'm happy to do multiple tattoos in one session!
-All of my flash sheets are labelled by letter and each design is numbered. Please include the information for which design(s) you are interested in (ex: F4, G13, etc.)
PRICING FOR ALL FLASH:
Designs with no asterisk start at $150, around 3 inches
*Designs start at $200, 4-5 inches
**Designs start at $250, 5+ inches
***Designs start at $300, 7+ inches.
Most designs can be sized up, however they cannot be sized down.
-I also have “Forever Flash” (Marked with ∞) Which are always available. These designs are also listed in my flash book in person, if you wish to put a $100 deposit down and decide on the day of your appointment.
-I require a 50% deposit in order to book your appointment, which goes towards the final cost of your tattoo. Your appointment is not booked until your deposit is recieved and confirmed by me. This deposit is nonrefundable, please make sure you are willing to follow through on your appointment before placing a deposit. Deposits will not be refunded once received.
-If you need to reschedule your appointment, please reach out more than 48 hours in advance so that I can attempt to fill the spot with another client. Cancelling/rescheduling within 48 hours of your appointment will forfeit your deposit.
-I will reach out to you via email to accept your appointment and provide info on sending your deposit. I accept Zelle, Venmo, or Cashapp. The remaining balance on the day of your tattoo can be paid via the above, or in cash. Please avoid putting things like "tattoo" or "deposit" in the caption ("Thank you!" or emojis are great)
-Tips are not required but greatly appreciated, and range from 15%-25% if you had a great experience :)
